This temporary website provides clinic information. Patient appointment and medical-message forms are disabled until the clinic connects and approves a secure system. Do not submit medical records, diagnoses, symptoms, test results, insurance member numbers, Social Security numbers, or other sensitive health information through ordinary email or public website forms.
Information submitted through forms
The volunteer-interest form is for non-patient administrative information only. Do not use it for patient care, appointments, medical records, or health questions.

Donation processing
Online donations are processed by the donation provider connected to the website. Payment-card information should be entered only into the secure provider form and is not intended to be stored in ordinary WordPress form messages.
